How to Get Back on Health Insurance After Quitting in Japan
Quit your job in Japan? Compare NHI, optional continuation, and dependent coverage before the 14-day deadline creates trouble.

Deadline: if your employee coverage ends and you need National Health Insurance, many cities tell you to notify your ward or city office within 14 days. If you want optional continuation through Kyokai Kenpo, the application window is 20 days after resignation.
Where to go: choose one path only and apply at your municipal National Health Insurance counter, your former insurer's optional continuation desk, or your family member's employer for dependent coverage.
Bottom line: do not sit at home waiting for HR. If your documents are delayed, cities like Osaka say to consult within the same 14-day window anyway.

If you are searching for health insurance after quitting job Japan, the biggest mistake is assuming somebody else will fix it for you. When I lived in the UK, I could not even register with the NHS over the phone without multiple failed calls. Later, while working as a Medical Coordinator for Foreign Patients in Osaka, I saw the same thing in Japan again and again: the problem was not a lack of systems. It was a lack of access.
This article is not a generic insurance explainer. It is the fast resignation playbook I wish more foreign residents had before they ended up uninsured, back-billed, or asked to repay medical costs because they used an old company card.
Start with the hard numbers. These are the dates that actually matter.

Do you need NHI, optional continuation, or dependent coverage?
Your real decision point is the day after your employee coverage stops. Yokohama says National Health Insurance starts from the day workplace insurance ends, usually the day after retirement, and Shinjuku says your coverage begins from the day you should have enrolled, not the day you finally file.
- Choose National Health Insurance first if you are not already accepted into another plan. This is the default route for residents who are not on workplace insurance, later-stage elderly coverage, or welfare.
- Choose optional continuation if you were on employees' insurance and want to keep that style of coverage. Kyokai Kenpo says you need at least two continuous months of insured status before retirement, must apply within 20 days, and can keep it for up to two years. The premium is usually close to double what came out of your paycheck because the employer share disappears.
- Choose dependent coverage if a spouse or family member has workplace insurance and you actually qualify. Exact rules vary by insurer, but Kyokai Kenpo still uses the classic under-¥1.3 million annual income test in many cases, and from October 1, 2025 some non-spouse dependents aged 19 to 22 moved to a ¥1.5 million threshold. Confirm with the family member's HR before assuming you are eligible.
If optional continuation or dependent coverage is not already confirmed, NHI is the safer fallback. In practice, it is better to enroll and later withdraw once your other plan is active than to leave a gap and discover your clinic visit fell into an uninsured period.
How to enroll at city hall within 14 days
This is the part most people overcomplicate. Osaka says the key proof after leaving company coverage is a health insurance qualification loss certificate or a Mynaportal medical insurance screen. Yokohama also explains that, depending on your Myna registration, the office now issues either a Notice of Eligibility Information or an Eligibility Confirmation Certificate instead of the old paper insurance card.
- Your residence card or My Number Card
- Your health insurance qualification loss certificate (健康保険資格喪失証明書, Kenkou hoken shikaku soshitsu shoumeisho) or a Mynaportal eligibility screen/printout
- Cash card, bankbook, or seal if your city asks you to set up payment details

- 退職して国民健康保険に入りたいです (Taishoku shite kokumin kenko hoken ni hairitai desu) — I quit my job and want to enroll in National Health Insurance.

As one useful city example, Shinjuku started electronic qualification gain and loss procedures on March 17, 2025 for certain cases. That does not make the national rule different, but it is a good reminder to check whether your own city offers online or postal filing before you take time off work.
What to do if your employer delays the loss certificate
Do not wait passively. Osaka specifically says that if your required papers are not ready within 14 days, you should still contact your ward office within that same period and consult.
- Return the old company card immediately. Kyokai Kenpo says you lose eligibility from the day after retirement, and using the old card can lead to a later claim for 70% to 80% of the medical cost.
- Ask your employer in writing for the health insurance qualification loss certificate (健康保険資格喪失証明書, Kenkou hoken shikaku soshitsu shoumeisho). Keep screenshots or email trails.
- Go to city hall anyway within 14 days and explain that the certificate is pending. If you have Mynaportal proof, bring that.
- If you want optional continuation, move separately. Kyokai Kenpo says a certificate of resignation or retirement can speed issuance of the optional continuation card when you are in a hurry.

Late filing does not save money. Shinjuku says eligibility starts from the day you should have enrolled, and Osaka says if your NHI enrollment notice is late, the city can bill premiums retroactively for up to two years.
NHI premiums vary by municipality, and official city guidebooks include income-based reductions for lower-income households. If your unemployment was involuntary, MHLW says eligible Hello Work leavers can have NHI premiums calculated using only 30% of prior salary income, generally until the end of the following fiscal year. Ask your city hall what they need from Hello Work before the first full bill arrives.
If you do not pay, the problem can escalate. Osaka says arrears can trigger late charges, asset investigation, seizure of bank accounts or wages, and in longer-running cases a special treatment route where you pay 100% upfront at the clinic and claim reimbursement later. And separately, Kyokai Kenpo warns again that using your old company card after resignation can produce a later repayment demand.
