Japan Myna Hokensho 2026: The Real Transition Story for Foreign Residents

Four months in, the reality is messier. Adoption sits in the low-to-mid 60s percent range, card readers still break, name mismatches plague foreign-resident records, and many hospitals are quietly still accepting expired paper cards through a temporary extension that runs until July 31, 2026. This guide explains how the system actually works right now, what to do at the reception desk when things go wrong, and the foreign-specific issues that don't appear in official guides.
The three documents you might have
Post-transition, any given foreign resident carries one (or more) of these:

How to register your Myna card as an insurance card

資格情報のお知らせ — the paper you must not lose
If you did register your Myna card for insurance, your insurer mailed you a separate document called the 資格情報のお知らせ. This shows your insurer name, 記号 (symbol), 番号 (number), and other identifying data. Keep it — and bring it to hospital visits alongside your Myna card.
Why:
- When a hospital's card reader breaks or the network is down, reception staff use the 記号・番号 from this paper to look up your record manually
- Government procedures unrelated to hospital visits (家族の国保脱退手続, 医療証申請, 介護認定, 保険給付申請) still require the 記号・番号
- MHLW issued renewed guidance in November 2025 after many patients showed up with only the 資格情報のお知らせ — that document alone is not valid for clinical visits; it must pair with the Myna card
A screenshot on your phone is not a valid substitute — it must be the paper original or a printed copy from Myna Portal.
What happens at hospital reception when things go wrong
Even four months after the transition, common problems at the reception desk include:
The card reader fails or the network is down
Reception staff take your Myna card and 資格情報のお知らせ, then run online 資格確認 manually from the back-office terminal. This takes 2–5 minutes. You receive normal 7:3 billing.
You haven't registered your Myna card yet
If you only have the Myna card without insurance registration, you will likely be asked to pay 10 割 (full price) upfront. Later, submit receipts to your insurer for 7 割 reimbursement via 療養費支給申請. This can also be avoided by bringing your 資格確認書 — carry it separately until registration is complete.
Your insurance status hasn't updated yet after a job change

Your name on the card doesn't match hospital records

When the card reader returns your name, the reception system cannot match it to your existing patient record. You will be asked to confirm personally — and you may be registered as a new patient, losing access to past medical history at that clinic.
Fix: before your next visit, ask the hospital's front desk to update your record to match your 在留カード name exactly.
Losing your Myna card or traveling abroad
Lost or stolen
Call the Myna Number Comprehensive Freedial: 0120-95-0178 — 24 hours a day. From abroad: +81-3-6734-0170. The line suspends your card's digital certificate immediately. Report the physical card loss to your local 警察 within a few days — you need the 遺失届受理番号 to reissue the card at your city hall. Reissuance typically takes 1 month; during that window, request a temporary 資格確認書 from your insurer.
Traveling abroad — foreign nationals
Here is an important asymmetry: since May 2024, Japanese nationals who submit 国外転出届 can apply for continued use of their Myna card abroad. Foreign nationals cannot. When you submit 国外転出届, your Myna card is invalidated at your planned departure date. When you return to Japan and register a new 住民票, you must re-apply for a Myna card.
If your travel is short (not accompanied by 国外転出届), your Myna card remains valid throughout the trip.
Medical care while abroad
Myna 保険証 does not work at foreign hospitals. Pay in full locally, keep all receipts and diagnostic documentation, and file a 海外療養費 claim with your insurer within 2 years after return. This process is unchanged from the pre-Myna era.
Foreign-specific issues not covered in the official guides
在留カード number and Myna number are different
The 12-digit Myna number does not change when your 在留期間 is renewed and your 在留カード番号 changes. The two numbers are separately tracked. There is ongoing work to unify them under a single 一体化カード (law passed, implementation in progress — no firm date as of April 2026). Until then, make sure both numbers are up to date on your records.
Name mismatches from 通称名 or 漢字表記 requests
If you have registered a 通称名 (commonly used name) or a 漢字氏名表記 (available for nationals of China, Korea, and other kanji-culture countries via ISA kanji name application), your Myna card can display either form. But the insurance record linked to the card uses whichever name was on your 在留カード at time of enrollment. Picking up a new Myna card with 漢字表記 while your hospital records still reference your ローマ字 name guarantees a mismatch. Before updating, tell your hospital.
Myna card expiry = 在留期間 expiry
For foreign nationals, the Myna card's validity equals your 在留期間 expiration date. When you renew your 在留期間, you must also renew the Myna card at your city hall — do not wait until it's needed at the hospital. Without a valid Myna card, your insurance registration suspends.
Address change — the 14-day rule still applies
Within 14 days of a move, update your 住民票 and update the 券面記載 on your Myna card at the municipal office. Until the update, 資格確認 may show your old address, causing discrepancies at new hospitals.
The transition safety net — what expires in July 2026
MHLW extended the post-December 2025 grace period to July 31, 2026. During this window, hospitals can accept:
- Expired paper 健康保険証
- Myna card plus old 資格情報のお知らせ (when reader fails)
- 資格確認書 (continuously — not dependent on the grace period)
After August 1, 2026, the paper-card fallback ends completely. At that point, every patient must present either a functioning Myna 保険証 registration OR a valid 資格確認書. Foreign residents who have not dealt with this yet have until end of July 2026 to get a 資格確認書 mailed or register their Myna card.

A decision framework for foreigners
- Have a Myna card and comfortable with digital procedures? → Register via Myna Portal or at your next hospital visit. Keep the 資格情報のお知らせ paper.
- Have a Myna card but prefer a paper backup? → Register for Myna insurance AND request a 資格確認書 (available on application).
- Don't have a Myna card / don't want to register? → Your 資格確認書 comes automatically. Use it like the old paper card.
- Your insurance status just changed (new job, quit job, moved city)? → Wait 5 business days, then verify your new 資格情報のお知らせ has arrived. Until then, use the old paper + Myna card combo at hospitals (valid until July 31, 2026).
- Planning to leave Japan permanently? → Cancel 住民票 at the city office; Myna card invalidates at departure. Re-apply when/if you return.
